Apply for Certification by Mail

Please review the eligibility criteria in the General Application Guide before beginning your application. Special criteria apply to enterostomal therapy applicants.

Four Easy Steps

• Download or obtain the General Application Form

• The downloadable form is in PDF format. It can be saved to your computer and filled out onscreen, but information entered and modified cannot be saved. However, it can be printed.
• You can request to receive printed copies of the form by mail by calling the CNA Certification Program Processing Centre at 1-800-450-5206.
• o You can also complete a Request for Information form.

• Complete the general application form and other associated forms

• Refer to the General Application Guide for details.

• Print and mail the completed forms

• Mail it to the address on the form and keep a copy for your records. You cannot submit the form electronically.
• Mail a stamp if you would like CNA to send you a notice card to confirm receipt of your application. (This card is not available to online applicants or applicants who download the form.)

• Wait for eligibility verification

• CNA staff will review your application to determine your eligibility.
• Once eligibility is determined, an e-mail confirming your eligibility, followed by a confirmation letter — both of which contain your link to access the online exam prep resources – will be sent to you.

Changes to contact information can also be e-mailed to CNA or can be made by completing this online form.
